7/8/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Anthony upon arrival to the home setting. Upon arrival to the home setting, Anthony appeared to be in a good mood. Family was receptive as family indicated that they are grateful to continue to receive services during this pandemic. Anthony greeted QMHP-C upon arrival to the home setting. Upon arrival to the home setting, Anthony appeared to be in a positive mood due him engaging in general conversation with QMHP-C. 2. Behavior: Anthony presented cooperative and normal behaviors as he did not need any redirection from QMHP-C as well as him answering open -ended questions and being praised by QMHP-C. 3. Behavior: Anthony was withdrawn during this portion of the session as he was playing with his younger sibling. After redirection from QMHP-C for him to gain focus of the session he was able to engaged back in the session. 1.Intervention: QMHP-C assigned one positive affirmation statement to say this week. QMHP-C engaged Anthony in a mood exercise to identify areas he most struggle. QMHP-C the processed and explored further with Anthony. QMHP-C taught effective ways to communicate her feelings and express his thoughts accordingly through role-playing and being more assertive when communicating with others. 2. Intervention: QMHP-C assisted Anthony in gaining insight how worry is a form of avoidance and fear which creates problematic behaviors for him when he becomes emotional. QMHP-C engaged Anthony in thought-stopping by replacing negative thoughts into positive before acting out sporadically. 3. Intervention: QMHP-C provided a therapeutic activity on managing anger. QMHP-C assisted Anthony with adequate ways to manage his anger across all settings such as school, home, and in the community. QMHP-C real life scenarios to model and suggest to Anthony constructive ways to handle his anger with siblings and peers. QMHP-C taught Anthony about being empathetic to victims (siblings/peers) of his bullying and intimidating behaviors. 1. Response: Anthony appeared to be receptive to QMHP-Cs’ interventions and services that were provided during the session. Anthony appeared to be receptive to QMHP-Cs’ interventions and services that were provided to him during the session. Anthony reported that he has not follow through on the assignment and shred that he only did it once when QMHP-C demonstrated. 2. Response: Anthony reported that he was worried that his mother would start ignoring him and not listening to him and what he wants. Anthony discussed with QMHP-C that he understands that his mother is trying to protect him but at the time he needs to just understand that he too have friends that he likes to play with but he know that he cannot play with them because of his anger. 3. Response: Anthony had little insight into how his communication approach is towards others. Anthony does not understand the concept of making demands and requests. Anthony did begin to gain insight when some of the possible results that QMHP-C explained were discussed with him. Anthony continues to struggle with managing his anger but did share that he is now able to recognize factors that makes him angry.
